Dear Dr Scurlock,

I am the solicitor in conduct of a claim for an injunction against Mr Cordell and write in response to
your email to Kaunchita Maudhub on 15th June 2018.

I would need further clarification on the content of your email and hope you will be able to respond
as soon as possible as a hearing has been listed on Tuesday 26th June 2018.

1) You explained that you have been trying to see Mr Cordell but he has not attended the
assessments offered to him although he has phoned the assessment team.

Can you please confirm the dates of the assessments offered to Mr Cordell and details of the
conversations that were held?

2) You mentioned that he had contact with various mental health teams and had one brief
admission on the Haringey assessment ward in August 2016.

Will it be possible to have details of the contacts held and the assessment that was carried out. If
possible we would like a detailed history of the contacts/ admissions so we could provide the
information to the Court.

3) You suggested that a report is to be commissioned from a Consultant Forensic Psychiatrist
from North London Forensic Service as this is not something you would provide.

Can you please confirm what would the report achieve? If this is not a service you could provide,
why did you department contact Mr Cordell and offered him an assessment in the first place?
