I Mr Simon Paul Cordell am writing this letter to confirm I give Barnet, Enfield and
Haringey Mental Health NHS Trust authority if they need to speak or send any
documents relating to me, to my mother Miss Lorraine Cordell on my behalf in relation to
the subject access request I have submitted relating to obtaining all my medical health
records.

Barnet, Enfield and Haringey Mental Health NHS Trust can contact my mother Miss
Lorraine Cordell via the information below and may speak or send any documents to her
on my behalf, once the information is collected I have asked my mother to attend and
pick the information up and pay any fee needed on collecting the information. As I know
it will be safer for it to be picked up in person.

Can this letter please be placed on my file so that my mother has authority?
