We would be very grateful if you could provide us with details of health checks you have carried out for
this patient within the last twelve months with regards to:
• Cardiovascular Disease
• Chronic Obstructive Pulmonary Disease
• Diabetes
• Height & Weight
• Blood test results
It would be very helpful if you could also provide details of any other significant physical health
conditions the client has been diagnosed with, or is being investigated.
We will update you routinely and following reviews and I look forward to working with you to support Mr
Simon CORDELL with his mental health problems.
