Thank you for your letter dated 17th December 2015 requesting for any information
regarding any health checks done for the above patient within the last twelve months.
Unfortunately, this patient has not been seen in the surgery for more than one year. One of
our doctors actually called him but he did not want to speak to us and through the mother we
have Invited the mother to come in for his health checks and medical reviews but he has not
responded, so we are unable to provide you with any up-to-date details of his current physical
status.
